Anti-GQ1b IgG Antibody Syndrome Presenting as Pure Complete Ophthalmoplegia without Ataxia

Abstract
Immunoglobulin G (IgG) anti-GQ1b antibody is closely associated with acute paresis with extraocular muscles. Anti-GQ1b antibody has been found in Miller-Fisher syndrome, Guillain-Barre syndrome with
ophthalmoplegia, Bickerstaff brainstem encephalitis, and acute ophthalmoplegia without ataxia. Therefore, ophthalmoplegia associated with anti-GQ1b antibody should be considered in the differential
diagnosis of acute or subacute ophthalmoplegia. However, few studies described anti-GQ1b antibody-associated acute ophthalmoplegia without ataxia. We describe a patient who presented with bilateral pure complete opthalmoplegia with ptosis. The patient had significant elevation of anti-GQ1bIg G antibody titer without other neurological abnormalities.
